Episode 666: Protect Against Peri-Implantitis: Kill the Bugs in the Implant Channel

The Dr. Phil Klein Dental Podcast
Guest: Larry Clark CE Credits: 0.25 CEU
Release Date: 5/8/2025
Periodontics Implant Dentistry

What if a simple bacterial colonization in your implant channel could silently undermine years of successful implant therapy? Research reveals that micro-movement between abutments and implants creates the perfect breeding ground for pathogenic bacteria, leading to peri-implantitis rates as high as 10% in some studies.

Larry Clark brings over 50 years of dental industry experience as Director of Clinical Affairs and Marketing at Pulpdent. Since 1971, he has served in various capacities including Education Facilitator at the USAF General Dentistry Residency Program and multiple dental sales and product management roles. An enthusiastic dental educator, Larry has lectured globally on dental materials and clinical applications, authored numerous industry publications, and holds membership in several dental industry groups. In 2015, he became the first non-dentist, non-dental lab member inducted into the American Society for Dental Aesthetics.

This episode explores the hidden threat of peri-implantitis caused by bacterial colonization within implant channels. Larry discusses groundbreaking European research showing distinct bacterial populations in failed implants, including higher concentrations of Staphylococcus aureus not typically found in periodontal disease. The conversation reveals why traditional sealing materials like Teflon tape may actually attract biofilm formation and examines innovative antimicrobial solutions for long-term implant success.

Episode Highlights:

  • European studies demonstrate that 90% of implant patients experience some form of mucositis or peri-implantitis in 13-year follow-ups, with mean peri-implant prevalence rates around 10% across over 9,000 patients. Swedish government research reports implant failure rates in the 7-9% range, while UK studies show 94% survival rates but widespread inflammatory complications.
  • Micro-movement between abutments and implants creates gaps that allow bacterial infiltration into the implant channel, regardless of whether the restoration is screw-retained or cemented. Cone beam studies from Germany demonstrate implant flexion at the crestal surface during occlusal loading, creating a suction effect that draws bacteria into the channel.
  • Traditional Teflon tape sealing provides no antimicrobial protection and actually attracts biofilm formation. Research shows that vinyl polysiloxane and zinc oxide eugenol provide superior sealing compared to Teflon tape, though removal difficulties limit their clinical utility for maintenance procedures.
  • Bacterial populations in peri-implant sites differ significantly from periodontal pathogens, with notably higher concentrations of Staphylococcus aureus found in failed implants compared to healthy implants or teeth with periodontal disease. This distinct microbiome requires specific antimicrobial approaches rather than traditional periodontal therapies.
  • Nano-silver impregnated polymer plugs offer long-term antimicrobial protection with 6-10 year substantivity demonstrated in clinical studies. The silver remains bound within the polymer matrix, killing bacteria on contact without silver release, and the plugs can be easily removed and reused during maintenance procedures.

Perfect for: General dentists and oral surgeons who place implants, periodontists managing peri-implant complications, and dental teams seeking evidence-based approaches to implant maintenance and long-term success.

Discover how simple changes in implant channel management could dramatically improve your long-term implant success rates.
